There are seven (7) forms and additional supporting information that you will need to complete for your NEF Application:
1. The Application;
2. Professional Nursing Membership Verification;
3. National Student Nurses' Association Form;
4. and four (4) Reference Forms.

In addition, NEF requires official transcripts of all coursework; a copy of your valid state registration; and a copy of your birth certificate or biographic page from your US Passport (or see below).

The deadline for completed applications, including all support materials, is March 1st. If materials do not reach us by that date, your entire application will be declared ineligible. Please be sure your name appears on each page submitted AND that documents consisting of more than one page are stapled together.

The Application Form
Print or type all information on this Application Form. A copy of your current, valid state registration as a professional nurse must accompany this form.

Proof of Admission to a program of nursing education must be sent to NEF. At the master's degree level, this must be a professionally NLNAC or CCNE accredited program in nursing; at the doctoral level, this program may be in nursing or in a nursing-related field in an accredited institution. No funds will be awarded until the formal notice of admission to a program has been received.

A Goal Statement Essay stating your long-term professional goals and your potential for making a contribution to the nursing profession must also be included with your Application Form. Essays are read carefully by the Criteria and Eligibility Committee and by the NEF Membership before the selection of winners is made. Essays are rated on the quality of your long-term professional goals that you have clearly identified, on your self-assessment of your potential contribution to the nursing profession, and on the quality of the written English. Your essay should not be a recitation of your educational activities, but should show the raters your potential plans and activities and their value to the nursing profession and/or the care of patients. Essays must not exceed two (2) typewritten double-spaced pages.

Professional Nursing Membership Verification Form
Fill out the form as indicated and send to the appropriate national professional nursing association. They should be asked to use the form to verify your membership and they can send it directly to NEF or back to you to be included with the whole application.

The National Student Nurses Association Verification Form
This form is necessary only if you are/were a member of NSNA.

Four Reference Forms
References serve as a means of gathering information about an individual's academic and leadership potential, as well as personal and professional attributes. Reference forms should be given to individuals who know you professionally, academically, personally and as a member of the community. Preferably they should represent a variety of your associations not just one school, one employer, etc.

Additional Requirements
Applicants must provide proof of citizenship by submitting a copy of either: the biographic page of your United States Passport; a copy of your Birth Certificate; or a copy of your Naturalization Certificate from the USCIS.
All applicants should arrange to have official transcripts of all college and university credits sent to you to be included in the application or to NEF. In addition, send your most recent semester's grades, whether official or unofficial.

The prompt submission of all application materials is the responsibility of each applicant. NEF does not send notice regarding receipt of materials, however, NEF will notify you if any materials are missing from your application. No extensions will be given past the March 1st deadline.

Please enclose a non-refundable check in the amount of Twenty Dollars ($20.00) for the processing fee made out to NEF. All application materials become the property of NEF and are not returnable.
